Job Description: ****THIS POSITION WILL SERVICE OAKLAND COUNTY**** I. GENERAL STATEMENT OF RESPONSIBILITIES Monitor and document the whereabouts of delinquent youth in diversion and community reintegration program. Provides transportation for youth and families. II. REPORTING RELATIONSHIP A. Reports directly to: Community Support Supervisor B. Supervises: Not applicable III. QUALIFICATIONS Education: Bachelor's Degree Experience: Practical youth and education experience * Skills: * Ongoing and reliable transportation as required to make home visits and provide transportation to youth and families. Valid unrestricted Michigan driver's license. Good driving record. Provide proof of vehicle insurance coverage on the vehicle used for transporting youth and families * Satisfactory MDHHS Central Registry and Police Record LIEN Check. Negative TB test and satisfactory physical exam upon hire * Accurately composes and submits all reports, letters, and any other documentation meeting MDHHS and quality standards * Reads, stays familiar and complies with all Service Standards applicable to the intensive in-home service programs, e.g., Michigan Department of Health and Human Services Manual Items, Program Manual, Master Contract, Juvenile Court procedures, contract language etc. D. Abilities: Ability to develop positive relationships with youth and families. Serve as a constructive adult role model. E. State of Michigan Licensing requirements and driving record clearance. F. Valid, unrestricted State of Michigan Driver's License. Provide Proof of vehicle insurance coverage on the vehicle used for transporting youth and families. G. Demonstrate good moral character IV. SPECIFIC DUTIES AND RESPONSIBILITIES 1. Provide a wide range of goal-directed services to the youth and family to support the MDHHS case service plan and court orders and may include, but is not limited to: a. Aid the youth and family in developing a behaviorally-specific family or individual safety plan(s) b. Assess and teach appropriate parenting skills c. Teach anger management and communication skills d. Assess and teach budgeting skills e. Aid the family in meeting medical needs for children or adults in the family, including but not limited to, substance use disorder or mental health treatment and supports when appropriate f. Teach and model behavior within the family, including household roles and responsibilities and other organizational skills needed to provide a positive environment g. Refer and link the youth and family with necessary concrete services (such as furniture) h. Refer and link the youth and family to community and prosocial activities (such as organized sports) and supports and resources i. Assist the youth and family with accessing transportation and/or transporting the youth and family as needed 1) For reintegration services, this includes transporting or arranging transportation for the family to participate at least twice in visitation and family sessions during the last four weeks of residential treatment j. Provide court reporting/testimony, as needed k. Develop the Support plan goals and activities in coordination with the youth, family, referring worker and for reintegration cases, the treatment and transition team members l. Work collaboratively with all other service providers involved with the youth and family to assist the family with goal achievement m. Clean, paint, relocate or assist a youth and family in any means necessary during or after a crisis n. Teach life skills to the youth and family members throughout the intervention (e.g.: develop chore charts, daily routine plans, crisis cards and strategies of family activities) VI. OTHER DUTIES AND RESPONSIBILITIES Acts as positive role model for youth, family and the community. Acts as an advocate for all youth and families. Completes activity logs, mileage forms, petty cash and flexible monies forms, maintenance forms and time sheets clearly and accurately. Complies with all Spectrum personnel policies and procedures. Complies with program manuals and procedures. Attends and actively participates in all trainings, in-services, conferences and meetings as required by Spectrum and MDHHS. Completes all tasks and duties as assigned by Spectrum supervisory and administrative staff. The selected candidate must successfully complete a comprehensive background check including fingerprinting, medical clearance (TB test, drug screen and physical), DHHS clearance and MVR clearance.
