Job Description:
Position Overview: The Home Visitor supports high-risk pregnant
and postpartum individuals through outreach, home visits,
education, and referrals while managing a caseload of 35–40
clients. The role focuses on enrolling eligible women, developing
care plans, connecting families with services, and providing
follow-up support. This full-time position is grant funded and
continued employment depends on ongoing funding and demonstrated
competency. Key Responsibilities: Identify and recruit high-risk
pregnant women in Lucas County for program enrollment, maintaining
an active caseload of 35–40 pregnant and postpartum clients in
accordance with grant guidelines. Complete and maintain required
client records, ensuring accurate and timely documentation that
meets all program standards. Develop, implement, and monitor
individualized client care plans, providing referrals to
appropriate health and social service resources. Motivate and
support pregnant women in accessing prenatal and postpartum care;
coordinate connections with service systems and providers and
conduct follow-up to ensure clients receive needed services.
Promote healthy lifestyle choices by educating clients on
strategies to manage and reduce health risk factors. Provide
health, developmental, and educational information tailored to the
needs of the target population. Qualifications: High school
diploma/GED; Community Health Worker certification (or obtained
within one year). Valid Ohio driver’s license, auto insurance,
reliable transportation, and willingness to travel throughout Lucas
County. Culturally sensitive with a pleasant, mature, and
professional demeanor. Strong organizational, communication
(verbal/written), listening, and interpersonal skills. Ability to
follow instructions, use sound judgment, and work independently or
as part of a team. Patient, understanding, and able to handle
challenging clinical situations. Basic computer skills (Microsoft
Windows, Word). Maintains confidentiality and presents a neat,
