Care Clinical Manager, you’ll be the heartbeat of our clinical
operations. Your days will be filled with purpose as you: • Support
and mentor a dedicated team of nurses and caregivers • Ensure our
patients receive safe, high?quality, personalized care • Oversee
clinical documentation, care plans, and regulatory compliance •
Collaborate with physicians, referral partners, and families • Lead
case conferences and guide clinical decision?making • Help onboard,
train, and inspire new team members • Keep a close eye on quality
measures, patient satisfaction, and outcomes This role blends

license • 2–3 years of clinical experience in home health, hospice,
or a related field • Previous leadership or supervisory experience
(or a strong desire to grow into it) • Solid understanding of OASIS
and home health regulations • A calm, confident communication style
and a knack for problem?solving • Comfort with EMR systems and
clinical workflows • A genuine passion for supporting patients and
