Job Description:
The ACT RN is: An ACT mental health professional assesses,
plans, develops, coordinates, and directly provides flexible
treatment, rehabilitation and support services to individuals who
have the most serious symptoms of mental illness and who
consequently have the greatest difficulty with basic daily
activities such as keeping themselves safe, caring for their basic
physical needs, maintaining a safe and affordable place to live,
maintaining sobriety, and other activities of daily living. Rather
than brokering services, the team itself is the service delivery
vehicle, and assumes direct responsibility for providing the
specific mix of medically necessary services required by the
persons-served, if there is a need. The Team ensures that services
are available 24 hours per day, 7 days per week. This position
promotes recovery-based, person-centered, and culturally competent
practices. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: This is not

- Support the engagement of meaningful activities by coordinating
and monitoring service activities such as a job, school,
volunteerism, family caretaking, for persons served.
- Functions as a team member rather than an individual
practitioner, knowing and working with all persons-served and their
family to recommend strategies for recovery.
- Promote a recovery centered environment and provide referral to
appropriate community resources.
- Know the contract requirements for your program/service.
- In concert with the persons-served and other Team members,
develop Treatment Plans, using person-centered processes in
identifying needs and setting goals.
- Maintain consumer records-meeting all appropriate standards
according to company policies and all regulatory bodies.
- Ensure that 100% of his/her training requirements are renewed
prior to expiration date, as specified by jobs.
- Cross-train with other members of the ACT team to provide
direct services which include, but are not limited to:
rehabilitation, daily living activities, family supports (crisis
management-supporting parents-counseling family members), health
living activities, counseling, therapy, entitlements, access
management, medication support, housing assistance, financial
management, counseling therapy, access to community resources like
food, clothing, developing natural resources and making contacts
with social support networks.
- Coordinate and monitor services through comprehensive tracking
of consumer activities and meeting with the consumer face-to-face
several times per week.
- On a rotation basis, provide on-call crisis intervention
services covering nighttime hours, as well as serve as a back up to
evening and weekend staff.
- Participate in providing substance use and treatment
services.
- Complete and submit Service Activity Logs (SAL's) in accordance
with agency policy/protocols
- Manage the medication system including MAR oversight,
medication storage, procurement, and drug destruction, as
ordered.
- Screen and monitor consumers for medical problems and/or side
effects.
- Engage in health promotion, prevention, and education
activities including those surrounding pertinent psychotropic
medications.
- Assist the psychiatric prescriber with upholding Hope Network
policies and procedures surrounding safe and informed psychotropic
prescribing, within their scope of practice. This includes
assisting with the process of medication prior authorizations as
needed, assist with refills, order labs according to protocol, and
assist with tracking and coordinating completion of consumer blood
draws, as able.
- Maintain CARF and regulatory standards including OSHA
requirements.
- Withing the office and community, administer and document
medication treatment including injections.
- Conduct consumer medical assessments and treatment appropriate
screenings i.e., metabolic syndrome. Alongside the consumer and
interdisciplinary team, assist with the development of wellness and
health management skills within the treatment plan goals i.e.,
nicotine cessation. Position Requirements:
- Minimum - current license to practice as a registered nurse in
Michigan.
- One Year or more of experience in a clinical or rehabilitation
setting with persons with severe mental illness
- Maintain a valid motor vehicle operator license with driving
records acceptable according to Hope Network policy.

- Registered Nurse (RN) -ability to be on call 24/7 and take on
call approximately 6 days per month
- All ACT staff must obtain a basic knowledge of ACT programs and
principles acquired through participation in MDHHS-approved
ACT-specific initial training, and subsequent participation in at
least one MDHHS-approved ACT-specific training annually thereafter.
All initial training of ACT staff must occur within six months of
hire for work in ACT.
