THE PURPOSE OF YOUR JOB POSITION
The primary purpose of your job position is to provide your
assigned residents with
routine daily nursing care in accordance with established nursing
care procedures, standards of practice and as may be directed by
your supervisors.
II. JOB FUNCTIONS
Efforts have been made to make your job description as complete as
possible.
However, it in no way states or implies that these are the only
duties you will be
required to perform. The omission of specific statements of duties
does not exclude
them from the position if the work is similar, related, or is a
logical assignment to the
position.
1. Record all entries on flow sheets, notes, charts, kiosks, etc.,
in an informative and
descriptive manner.
2. Immediately report all accidents and incidents to your immediate
supervisor that you have observed on the shift that they occur, but
no later than the end of your scheduled shift.
3. Report all changes in the resident's condition to the
Charge/Staff Nurse as
soon as practical.
4. Receive the nursing report upon reporting on/off duty and
maintain other
records as directed.
5. Perform and/or assist resident with all hygienic and grooming
functions,
including dental and mouth care, bathing, dressing, shaving, bowel
and
bladder functions.
6. Change bed linens and make beds.
7. Report character of stools and collect specimens as instructed
(i.e., urine,
sputum, stools, etc.).
8. Check residents routinely to assure that their personal care
needs are
being met per the plan of care.
9. Lift, turn, move, position, and transport residents into and out
of beds,
chairs, bathtubs, wheelchairs, lifts, etc.
10. Assist in transporting residents to/from appointments,
activities and social
programs, etc., as necessary, and accompany residents, as assigned,
to
scheduled appointments outside facility.
11. Assist residents to walk with or without self-help devices, as
instructed,
and assist with rehabilitative care, as directed and per the plan
of care.
12. Provide post-mortem care as instructed.
13. Observe and report to the Nurse in Charge, the presence of
reddened areas and skin breakdown to prevent decubitus ulcers.
14. Provide daily indwelling catheter care and daily perineal
care.
15. Provide daily Range of Motion Exercises.
16. Turn bedfast residents at least every two (2) hours or as
directed by the plan of care.
17. Provide residents with Social Interactions.
1. Prepare the resident's room for receiving the resident (i.e.,
bed made,
name tags up, admission kit available, etc.).
2. Participate in admission, transfer and discharge functions
pursuant to
Facility policy and supervisory instructions.
1. Prepare residents for meals (i.e., take to bathroom, wash hands,
comb
hair, raise bed, position tables, place clothing protectors, take
to/from
dining room, etc.).
2. Serve food trays. Assist with feeding as indicated (i.e.,
cutting foods,
feeding, assist in dining room supervision, etc.).
3. Record the residents food/fluid intake. Report changes in the
resident's
eating habits to the Nurse in Charge.
4. Perform after meal care (i.e., remove trays, clean resident's
hands, face,
clothing, take to bathroom, brush teeth, clean dentures, etc.).
5. Check rooms for food articles (i.e., food in proper container,
unauthorized
food items, etc.).
1. Attend and participate in scheduled training and educational
classes as required.
2. Attend and participate in scheduled orientation programs and
activities.
3. Wear name tag on your clean scrub uniform as required along with
no artificial nails.
1. Notify the Charge/Staff Nurse of any resident leaving/missing
from the
Facility and follow the instruction in the emergency manual on the
unit.
2. Follow established Infection Control & Universal Precautions
policies and
procedures when performing daily tasks.
3. Keep residence living units clean and safe.
4. Report any contagious diseases that you contact to the Director
of Nursing
Services and/or to the Infection Control Coordinator.
5. Provide illness symptoms when calling in sick in order to track
and prevent illness.
6. Ensuring Resident Safety.
1. Report defective equipment to the Charge/Staff Nurse.
2. Inform the Charge/Staff Nurse of your equipment and supply
needs.
1. Inform the Charge/Staff Nurse of any changes in the resident's
condition
so that appropriate information can be entered on the resident's
care plan.
2. Review daily nursing assistant assignments and Kardex
instructions of your assigned residents.
1. Maintain the -CONFIDENTIALLY -of all resident information.
2. Never take pictures inside facility without permission from the
Administrator.
2. Insure that you treat all residents fairly, and with kindness,
dignity, and
respect.
3. Report all grievances and complaints made by the resident to the
Nurse in Charge.
I. -Other Functions of a Nursing Assistant
1. Present a positive image of the facility to all customers, such
as job
applicants, families, and the community.
2. Deal honestly, openly, and directly at all times with all
Allegra Nursing and Rehab
personnel, expressing your views only in a constructive
fashion.
III. WORKING CONDITIONS
A. Works throughout the facility.
B. Sits, stands, bends, lifts and moves during working hours.
C. Is subject to frequent interruptions.
D. Is involved with residents, personnel, visitors, government
agencies/personnel, etc., under all conditions and
circumstances.
E. Is subject to hostile and emotionally upset residents, family
members,
personnel, and visitors.
F. Works beyond normal working hours, weekdays, weekends and
holidays,
when necessary.
G. Is subject to odors throughout the work day.
H. May be exposed to infectious waste, diseases, conditions, etc.,
including
the potential exposure to the -AIDS and Hepatitis B viruses.
I. Must be able to lift, push, pull, and move objects and patients
to the extent
necessary to perform the functions listed above.
J. May be required to assist in the evacuation of residents.
V. NURSING ASSISTANT EXPERIENCE
Must be a licensed Certified Nurse Aide in accordance with laws of
this state.
VI. PERSONAL, INTERPERSONAL COMMUNICATIONAL REQUIREMENTS
A. Must be able to read, write and speak the English language in an
understandable
manner.
