Summary
Speech-Language Pathologist Position Summary:
The Speech-Language Pathologist provides professional and
comprehensive care within the scope of agency services. The target
populations for this position are those requiring skilled clinical
home care services. Activities of this position are patient focused
and utilize assessment, planning, intervention, and evaluation in
accordance with speech-language standards, physician orders, and
agency policies and procedures.

A day in the life as a Speech-Language Pathologist with MyMichigan
Health:
- (25%)* Evaluates patient's functional status by performing
formal/informal assessment measures to determine: speech, language,
voice, cognition, and deglutition status.
- (25%)* Develops a goal specific treatment regime based on
assessment data and the patient's tolerance to carry-out the
treatment plan as prescribed.
- (25%)* Submits a written treatment regime to be included as
part of the plan of care authorized by the physician.
- (25%)* Devises a schedule of treatment which is frequency and
duration specific to be discussed with patient/family/caregiver and
submits to the scheduler.
OTHER DUTIES AND RESPONSIBILITIES:
- Uses the most appropriate and available modalities to assist
the patient in achieving functional goals.
- Adapts clinical treatment techniques and procedures to the home
environment.
- Recommends adaptive/supportive equipment based on assessment;
contacts appropriate representatives to place order and assists in
securing equipment for patient.
- Provides treatment techniques to facilitate, restore, or
maintain optimal functional performance through the following
modalities: Instruct, educate and/or train in therapeutic exercise
regime to increase oral motor strength and range of motion.
- Instruct, educate, and/or train in facilitation of cognitive
skills and deglutition skills.
- Recommend, instruct, educate, and/or train for augmentative or
alternative communication use.
- Demonstrates and instructs patient/family/caregiver in home
education program.
- Documents the patients/family/caregiver response to treatment
and changes in patients conditions per Agency policy.
- Coordinates patient care with the interdisciplinary team
members and other professional disciplines.
- Provides supervision for CFY Speech-Language Pathologist in
accordance with Agency and regulatory standards.
- Provides supervision for Home Health Aides carrying-out
treatment regime per Agency and regulatory standards.
- Provides consultative services to Agency staff members.
- Participates in interdisciplinary team conferences, in-service
programs, discharge planning, departmental staff meetings, select
task force committee meetings, and peer review process, assists in
program development upon request of Therapy Coordinator, and
represents Agency for community functions.
- Assists with student affiliate mentorship.
- May function as case manager for assigned cases.
- Other duties as assigned.
REQUIRED EDUCATION, EXPERIENCE, TRAINING AND SKILLS:
- Basic Life Support Required.
- State of Michigan Drivers License and Vehicle Insurance
Required.
- Fingerprinting Required.
- Speech Language Pathologist Clinical Competence
Certification.
- State of Michigan Speech Language Pathology License
required.
- Masters Degree Required.
- May consider a candidate who meets educational requirements for
certification and is in the process of accumulating the supervised
experience required for certification.
- Access to reliable transportation.
- Ability to communicate effectively, verbally and in
writing.
- Ability to recognize needs and concerns of people and maintain
constructive interpersonal relationships.
- Ability to work independently, demonstrating problem solving
and organizational skills.
- Ability to demonstrate knowledge, principles and practices of
speech-language pathology and related skills in patient
management.
- Ability to drive under hazardous road conditions.
- Ability to use personal protective equipment correctly.
- One year experience as a Speech-Language Pathologist,
preferably in homecare.
PHYSICAL/MENTAL REQUIREMENTS AND TYPICAL WORKING
CONDITIONS:Exposure to stressful situations, including those
involving public contact, as well as, trauma, grief and death. Able
to wear personal protective equipment that includes latex materials
or appropriate substitute if required for your position.Is able to
move freely about facility with or without an assisted device and
must be able to perform the functions of the job as outlined in the
job description. Overall vision and hearing is necessary with or
without assisted device(s).Frequently required to sit/stand/walk
for long periods of time. May require frequent postural changes
such as stooping, kneeling or crouching. Some exposure to blood
borne pathogens and other potentially infectious material. Must
follow MidMichigan Health bloodborne pathogen and TB testing as
required. Ability to handle multiple tasks, get along with others,
work independently, regular and predictable attendance and ability
to stay awake.Overall dexterity is required including handling,
reaching, grasping, fingering and feeling. May require repetition
of these movements on a regular to frequent basis. Physical Demand
Level: Light. Must be able to occasionally (0-33% of the workday)
lift or carry 11-20 lbs., frequently (34-66% of the workday) 10
lbs. and or Walk/Stand/Push/Pull of Arm/Leg controls.
