HealthCare Support is immediately interviewing for an RN Case Manager - Field role. This role presents the opportunity with an affiliate of a leading Fortune 100 company for a talented RN Case Manager to grow within a motivated team by demonstrating their direct patient care skills through home care visits, care planning, and effective communication. This is a full-time/direct hire position with full benefits! Daily Responsibilities for an RN Case Manager - Field:

• Assesses and evaluates patient needs to identify mutually agreed upon goals
• Develops patient care plans that specifically addresses identified patient needs and goals
• Supervises Home Health Aides and other skilled nursing support staff Requirements for an RN Case Manager - Field:
  • Active/Unencumbered RN license in the state of practice
  • Automobile to be used to work and current driver's license/proof of insurance
  • Clinical experience (home health or hospital) Hours for this Position:
    • Flexible - Monday through Friday - 8:00am - 5:00pm
    • On-call rotation: every 2-4 weeks (for 1 week); shift differential + pay per visit! Advantages of this RN Case Manager - Field:
