Summary
GENERAL SUMMARY
At the direction of case management staff, this position assists
with the implementation of the discharge plan as established by the
discharge planner and care team to support timely coordination of
care. The care coordinator must be able to collaborate with the
discharge planner to proactively identify and initiate role duties
to ensure timely facilitation of next site of care needs. This
position provides excellent customer service to patients through
timely follow up on post-discharge needs and ensures communication
with the patient and multi-disciplinary team. All staff in this
position will float across units, and assignments will vary based
upon department needs.
ESSENTIAL DUTIES AND RESPONSIBILITIES

Within scope of the role, uses critical thinking skills, decisive
judgment and the ability to work with minimal supervision.
Must be able to work in a fast-paced environment and take
appropriate action.
Look up information in the computer to answer inquiries,
documentation interactions and follow-ups, and send referrals as
indicated for the patient's needs.
Conducts phone calls and accesses systems to retrieve patient
information for clinical care team members and sends appropriate
information for placement or transfer.
Electronically faxes internal referral information as
indicated.
Works with internal and external resources to coordinate placement,
discharge planning, and follow-up care.
Other duties as assigned.
REQUIRED EDUCATION
High School Diploma or GED
EXPERIENCE, TRAINING AND SKILLS
Knowledge of reading, writing, mathematical, and basic computer
skills.

Completion of a medical terminology course preferred.
PHYSICAL/MENTAL REQUIREMENTS AND TYPICAL WORKING CONDITIONS
Exposure to stressful situations, including those involving public
contact, as well as, trauma, grief and death. Able to wear personal
protective equipment that includes latex materials or appropriate
substitute if required for your position. Is able to move freely
about facility with or without an assisted device and must be able
to perform the functions of the job as outlined in the job
description. Overall vision and hearing is necessary with or
without assisted device(s). Frequently required to sit/stand/walk
for long periods of time. May require frequent postural changes
such as stooping, kneeling or crouching. Some exposure to blood
borne pathogens and other potentially infectious material. Ability
to handle multiple tasks, get along with others, work
independently, regular and predictable attendance and ability to
stay awake. Overall dexterity is required including handling,
reaching, grasping, fingering and feeling. May require repetition
of these movements on a regular to frequent basis. Physical Demand
Level: Sedentary. Must be able to occasionally (33% of the workday)
lift or carry 0-10 lbs.
